kamailio fixes and enhancements for v5.0

• BUGFIX: Adding support for kamailio 5.X. Kamctl now reports uptime in format up_since, therefore monitor command needs to be updated
• Enhancement: Adding possibility to provide extra parameters in kamailio config files. This is needed in advance scenarios, where TLS, memory options, etc. are needed.
