Refactor: style: non-functional macros uniform&discernible (no include)

With an exception of "cib_channel_{ro,rw,shm}", the only macros
that got uppercased are located outside of "include" top-level
directory, and this exception was only made for uniformity with
CRM_SYSTEM_* symbolic IPC identifiers plus "internal.h" name already
signifies that's something not to be relied upon externally.

Besides, some macros were intentionally skipped from unification for
now (e.g. tools/crm_mon.c), and some ditched for being unused in
the process.
