Refactor: pacemakerd: use pcmk__server_ipc_name()

... instead of redundantly tracking the name in pcmk_children[].

This does cause pacemakerd to monitor pacemaker-based using
PCMKSERVER_BASED_RW instead of PCMKSERVER_BASED_RO, but that has no effect.
